Aspiration cytology, also known as Fine Needle Aspiration Cytology (FNAC), is a diagnostic procedure in which a thin, hollow needle is inserted into a mass, lump, or organ to extract cells for microscopic examination.

Purpose:

To diagnose lumps or swellings (especially in the thyroid, breast, lymph nodes, etc.).

To differentiate between benign (non-cancerous) and malignant (cancerous) lesions.

To detect inflammatory, infectious, or cystic conditions.

Procedure:

1. The area to be examined is cleaned and sometimes numbed.

2. A fine needle (usually 22–25 gauge) attached to a syringe is inserted into the lesion.

3. Negative pressure is applied to aspirate cells into the syringe.

4. The aspirated material is then smeared onto glass slides, stained, and examined under a microscope.

Advantages:

Quick and simple procedure.

Minimally invasive (no major surgery).

Low cost and few complications.

Gives rapid diagnosis for clinical management.

Limitations:

Cannot always give definitive diagnosis (especially if sample is inadequate).

Does not show tissue architecture (unlike biopsy).

May need confirmation by histopathology in doubtful cases.

Thyroid gland

Breast lumps

Lymph nodes

Liver and lung lesions

Salivary glands

Common Stains Used:

May–Grünwald–Giemsa (MGG) stain

Papanicolaou (Pap) stain

Hematoxylin and Eosin (H&E) stain

 Interpretation:

Cytological findings are categorized as:

Benign

Malignant

Atypical/Suspicious

Non-diagnostic/Inadequate sample


Benign (in Aspiration Cytology)
Meaning:
A benign lesion is non-cancerous. The cells do not invade nearby tissues or spread to other parts of the body.

Cytology Features of Benign Lesions:

Cells look normal or only slightly abnormal.

Uniform size and shape of nuclei (no pleomorphism).

Well-differentiated (cells resemble normal tissue).

No necrosis or mitotic activity.

Clean background in the smear.

Examples of Benign Conditions Diagnosed by Aspiration Cytology:

Thyroid: Colloid goiter

Breast: Fibroadenoma

Lymph node: Reactive hyperplasia

Salivary gland: Pleomorphic adenoma
